Goal: The Mobile C.A.R.E. Foundation’s mission is to provide complimentary and comprehensive asthma care and education to children and families in Chicago’s underserved communities via mobile medical units called "Asthma Vans."

Impact: The Mobile C.A.R.E. Foundation’s Asthma Vans provide children and families in Chicago’s underserved communities with complimentary and comprehensive asthma care and education resulting in reduced school absenteeism, decreased ER visits and lower hospitalization rates.

Goal: The Northern Michigan Diabetes Initiative is a regional collaboration dedicated to prevention, early detection, and management of diabetes. The Healthy Family Backpack Program connects with youth and their parents to educate participants on proper nutrition and promote healthy lifestyles to reduce childhood obesity.

Impact: The Northern Michigan Diabetes Initiative has distributed nutritional education materials to over 300 families. Of ninety-two families that set a healthy goal at the start of the program, forty-five continued to maintain that goal at the two-month mark.

Impact: The Community Preventive Services Task Force (CPSTF) recommends meal interventions and fruit and vegetable snack interventions to increase the availability of healthier foods and beverages provided by schools. This finding is based on evidence that they increase fruit and vegetable consumption and reduce or maintain the rate of obesity or overweight. Economic evidence shows that meal interventions and fruit and vegetable snack interventions are cost-effective.
