Goal: The goals of Engage DuPage include improving the health status of self-pay Emergency Department patients, increasing the revenue appropriately owed to hospitals for services rendered, improving the financial assistance processes involving self-pay Emergency Department patients, and reducing the number of avoidable Emergency Department visits among self-pay patients.

Impact: By the end of 2014, 57% of all benefit applications that were submitted were approved. In addition, 29% of all interested patients were connected to a primary care provider through a Community Access Specialist.

Goal: Every Baby to 1 works to ensure every infant survives and thrives during their first year of life. Every Baby to 1 engages the faith community to develop education efforts and initiatives that address infant mortality. As a part of this, Every Baby to 1 promotes the "3 S" approach of Safe Sleep, Safe Space, and Social Support to reduce the risk of Sudden Unexpected Infant Death (SUID).

Goal: To reduce the risk of falls among residents over 65 years of age through education in fall awareness and prevention.

Impact: After the program, participants spoke with healthcare providers regarding dizziness and medication side effects, scheduled overdue eye exams, and implemented home safety ideas to reduce the risk of falling.

Goal: The objectives of the FNHE program are to reduce incidents of chronic disease and improve nutrient intake and general quality of life by:
