This is a demonstration of the collaboration of multiple organizations  sharing a common goal and implementing services, programs and interventions that may ultimately  improve health outcomes. Organizations that provided outreach and educational materials are also listed below.

2020:
Habitat for Humanity 215 Individuals Impacted
Morris Habitat's Housing Solutions program helped 52 local families (170 individuals), provided furniture to two families (5 individuals), and provided laptops to 40 families. 
Narcan training, mental health services, recovery services, and treatment/rehab services. Individuals Impacted: 741
 
Saint Clare Behavioral Health 98 Individuals Impacted
Provided 98 assessments and 751 individual sessions,13 group sessions,10 family sessions and 294 faculty consultations (Output)
 
Child & Family Resources 39 Individuals Impacted/Certified
Mental Health First Aid Training  
 
Provide recovery support and education 
40 people received Mental Health First Aid Training Certification.
9 people received Narcan training.
