26 week Diabetes Prevention Program from Jan 10 - Nov. 13, 2019. 4 participants reached the weight loss goal of 7% or more, and all 8 participants reached the activity goal of 15 minutes or more per week.

Morris Somerset Regional Chronic Disease Coalition 362 Individuals Impacted

OUTPUT: • From January to March 2019 the RCDC conducted 3 outreaches, reaching 125 individuals. • From April to June 2019 the RCDC conducted 4 outreaches, reaching 109 individuals • From July to September 2019 the RCDC conducted 1 outreach, reaching 48 individuals • From October to December 2019 the RCDC conducted 4 outreaches, reaching 80 individuals. OUTCOME: 362 individuals were made aware of free cancer screening programs and were educated about cancer screening guidelines
